Can I have a ppp..

My little boy is 1 has got a temperature of anywhere around 38.9 and 39.2ish and medicine won’t take it below 38.9..when he’s awake he’s either a little bit wingy or very quiet and wants to sleep..but when i changes his bum I noticed this rash on his front..this is the best picture I can get but in person it’s more red. Do any mummy’s know what it could be please?
