Please can I have a PPP

I suffer with depression and anxiety and take medication (fluoextine) for and have for a number of years.
I go through good and bad patches.
Sometimes full of beans with ideas/energy etc then next day don’t have any energy or want to leave house.
I have 2 young children and try to make myself do things and go places sometimes but other times just love been at home all day doing nothing. I feel guilty that I should be out doing things if weather good (such as today when it’s sunny). Think I should be at park or something. I just simply cannot be bothered.
I am constantly making plans and then cancelling because I just ‘Can’t be bothered.
Not sure if i need to visit GP again, don’t think they can really do anything to help.
I do have a partner who works and is very supportive in fact he is only person I can talk to really as I hide it from other people quite well.
Sick of 101 things going round my head constantly. Is anyone else like this ever ??
Hate days like this and then give it a couple of days and I’ll be ok again but it’s only temporary. Then it starts again
