Ppp. Just looking for some advice please no bashing.

Today I received a phone call to say my daughter had been fed a glass pebble by another pupil. (Pic of pebble attached) she had swallowed it. She is 3 nearly 4. Anyway I took her to minor injuries and she had an x ray, thankfully it never got trapped in her lungs windpipe and was currently in her stomach. They said it will pass through and I have to watch her poo. Which is fine..
Now the hospital have now raised concerns as to why the children where un supervised whilst playing with these (they were role playing having a pic nic and another girl fed it to my daughter) they explained what would happen if it went it to her lung and worse case death 😢. Now I phoned my hv up and explained this and she said I need to speak to the school regarding policy to these pebbles and find out what exactly happened and why an incident form hadn’t been done. Fair to say I’m petrified after this has happened especially when the school have banned grapes and cherry tomatoes as a choking hazard 😕 but allow 3 and 4 year olds to play with pebbles unsupervised.
Spoke to the school and they said if they feel children can be trusted with these pebbles then they don’t have to be supervised, now am I been paranoid that I’m over reacting as they made me feel silly approaching them about this incident, at the end of the day I could of been going to a dead child. I’m concerned that this may happen to other children. I totally agree they allow pebbles for touch and paintings but surely they shouldn’t be left unsupervised as many children explore with the mouth. The teachers saw that they were playing with them so surely they should of been closely monitored. It was only by chance that the teacher looked over to the group of children that she saw my daughter being fed the pebble on a wooden spoon and then it didn’t come back out of her mouth. My hv and hospital both said a teacher should of been sat with them whilst playing with such small objects. Sorry for the long post and if it sounds mumbled just a very worried mummy. Am I over reacting or am I right to raise concern over this and with no incident form being done. Please no bashing.
